Student Affiliate Request

This form may be submitted by current employees of the University to request the creation of student affiliates on behalf of an academic program or office.

Instructions:

1. Download the Student Affiliate Request spreadsheet to the right

2. Fill in all of the requested information for each affiliate

3. Upload the spreadsheet into this form
