Advisor-Initiated Curriculum Change

 

Advisors may use this form to request curriculum changes, such as adding or dropping a major, concentration, minor, or certificate.

 

Note for licensure/certification programs:

The following steps apply when adding a program that leads to licensure or certification. Applicable programs are denoted by "(LIC)" within this form.

Step One: Advisor submits the Advisor-Initiated Curriculum Change form.

Step Two: The student is automatically notified by email that they must complete a Student Acknowledgement and Attestation for Professional Licensure Programs form.

Step Three: Upon receiving both the advisor's request and the student's attestation*, the Registrar's Office will process the curriculum change and send confirmation to the student and advisor.

*Both the advisor's and student's portions of the request must be received by the Registrar's Office on or before the 15th instructional day of the semester (or the equivalent thereof during the summer session).
