This form is for creating an email account request for active UM affiliates only, to be filled out by the sponsoring staff. Email account requests for staff should use UM email and computer account for employees. Approved affiliates of UM are eligible to have a UM email address and mailbox. The request must be completed by the affiliate's sponsor.
Affiliate email accounts are optional and should only be requested if needed for business purposes. An affiliate's email account is available for the duration of their relationship with UM.
