Network and telephone work order authorization

THIS IS NOT A REQUEST FOR A WORK ORDER TO REPAIR OR ADD NETWORK/TELECOM, IT IS FOR AUTHORIZATION TO SUBMIT THOSE WORK ORDERS

This form should be completed to authorize individuals within the unit who can initiate work orders and make changes to telephone and network billing.

If you are a manager you may authorize yourself on the form.

This form gives the named individual authorization to initiate work orders, update and make changes to telephone and network billing and data bases.  If there are multiple authorized users, please complete a form for each.

Please be aware only one user per index code can receive long distance billing statements. The system does not allow multiple contacts per index. We now distribute all long distance billing via e-mail once a month.

How to request

Please click  the “Request Service” button at the top right hand side of the screen.

You will be unable to access the Network Work Order Request and Telephone Work Order Request unless you are an authorized user.

You cannot request services from this page, this is for authorization only,  below are the links to each service.

Eligibility

All UM Employees may request to become authorized.

Availability

Form submissions available 24/7. However, authorizations will be processed on Monday-Friday, 8 a.m.-5 p.m.
